Case managers and discharge planners make the program available as a free-standing resource for stable patients rebuilding identity and direction after diagnosis or functional change. Sharing a resource link requires no health information and no clinical referral.

Vocational rehabilitation & independent living

Return-to-work starts with return-to-self. Vocational rehabilitation offices make the program available as a resource, and independent living centers offer it to the people they serve. It sits beside your services and is built to be doable on a hard day.

Employers & leadership

HR, benefits, and EAP teams share it with employees returning from medical leave or navigating a major life change. Training and speaking for leaders on disability-informed approaches and workplace accommodation are available.

Each partner follows its own compliance and endorsement rules. For publicly funded partners, you can begin simply by making the program available as an independent resource people can choose to use — no formal process required to share it.
